Medical Declaration

Before starting any in-water activities you need to have downloaded, filled in and returned to us the medical form.

If you answer YES to some of the questions you will need to get the form signed by a GP. If your local surgery don't offer this service then we highly recommend getting in touch with DDRC Healthcare. They provide diving medicals, often over the phone for a very small fee.
